Have You Received Your 1099-R Form?
By now you should have received your 1099-R form. If you still haven’t received yours, please contact our office. To learn more about this important form, please click here to view an informative video explaining the form and what to do if you do not receive it.


In the event that your 1099-R has an incorrect address, we ask that you download, complete and sign a Change of Address form (available here) and attach a note requesting a new 1099-R. You can mail or fax this information to our office.
