| 1099-R Forms Mailed Out |
1099-R forms have been mailed out to all participants. If you have not received your 1099-R by the middle of February, please contact our office.
In the event that your 1099-R has an incorrect address, we ask that you download, complete and sign a Change of Address form (available here) and attach a note requesting a new 1099-R. You can mail or fax this information to our office.
